Greetings, I am working as an HR in an IT-based application developing company, i.e., web application and mobile application. This is a startup company running for over four years. Until now, we don't have any standard format for appraisals. Now, our management has planned to implement a format that is applicable to all employees. So, please help me to create a standard format of the Appraisal form for Self-Appraisal and Performance Appraisal.

You just need to use the SEARCH tool located at the top middle of the page for your needs as there are a number of appraisal formats available. It is you who knows what you actually need, so I believe you can develop a good format or make the changes accordingly and use it.
