Form 7B of ESI is a medical acceptance card. This card contains the application by the IP to include his name in the list of doctors and the acceptance by the doctor. This card is available upon registering the IP online.

Medical Acceptance card (7b) is available on the ESIC website. You can log in with your credentials on the site, then print the counter foil tab. You should provide the IP Insurance No and submit; the left side will display the medical acceptance card format with pre-printed IP details. You can download it yourself and give it to the employee.

For Medical Benefits (CASHLESS) under ESIC, employees need to submit the following documents to the dispensary:
1. ESIC Card attested by the employer.
2. Temporary Identity Certificate/E-Pehchan Card with Aadhar card number attested by the employer.
3. Entitlement to Benefit Certificate through esic.in/employeeportal/login.aspx.
4. Eligibility Certificate issued by the employer.
