Hi, I am working as a Sr. HR Executive in a small IT company with an employee strength of between 40-50. I have to prepare a Performance Evaluation template and a questionnaire for yearly performance appraisal. It should be categorized into three evaluation tests:
1) Employee Self Evaluation
2) Manager's Evaluation
3) HR Evaluation
I need help and suggestions from all the HR professionals of Cite HR to prepare the same.

I would suggest going for an objective form rather than a subjective one; otherwise, the measurement part will be missing, and in the long run, that will affect the organization. Design one simple form with the parameters and then three columns (self-score, manager score, and HR score). Make it as fair and transparent as possible. Follow the below-mentioned steps:
1. The employee will fill out the form independently.
2. With the completed form, the employee will meet with the immediate manager to discuss the scores, and the manager will fill in the applicable column of scores.
3. If there is a difference of opinion between them, HR will intervene, and in a joint discussion involving all three parties, HR will fill in the final scores.
4. Take the average of the scores.
5. Follow the bell curve for any promotions, grade changes, and increments.
