One of my colleagues submitted the transfer form to the department, and he received a letter from EPFO stating that the forms need to be submitted along with EPF Annexure K. Please advise on how to obtain/send the EPF transfer Annexure K. Where can I obtain the same?

Ask you colleague to inquire to PF office for the said documents, I do not come across for this kind of form.

Dear Friend You have to submit Form 5A and Form 3A (Revised) for current year. The both forms can be collected from previous employer

I think they are confusing you because Annexure K is EPFO's responsibility.
I have also tried to transfer my previous PF amount to the current, but they said that we don't know about Annexure K. However, my transfer is okay when I met the senior EPFO employee.

Kindly resubmit the duly filled Form 13 to your new employer who, in turn, will send the same to RPFC along with ANN, "k", etc., as per provisions under the Act.
