I just wanted to know:
1) the process of registering a company for PF, which has an employee strength of 12, and company established for 6 yrs.
2) i know that form 1 is for registration process but the what is form 5 or 5A?and their purpose.
3) Should the employer/HR have to go personally to Regional office to submit the documents??
4) Is there any kind of fees we have to pay for registration?

The process for registration is as follows
1) Duly fill the application form "Form 1" as you discussed along with form 5A its called as business application form.
Along with that you should submit few supporting documents.
If Pvt Ltd company then should submit copy of AOA & MOA,
Board resolution
Company Address proof & copy of PAN
Directors Address & ID proofs
First invoice copy for proof of commencement date
Attendance sheet copy for last 2 months
Salary statement for last 2 months
Shops & Commercial establishments registration copy of factory licence
Copies of Any other registration certificate
Copy of telephone bill
etc.
As part of Fee there is no any fee. But they might ask you to pay around 2-3 months contribution as advance payment by DD and you shall adjust those payment for further periods.
Employer need not to go. Even HR shall go or you shall arrange even consultant for that.
Finally Inspector may demand some amount to get the certificate
