Dear HR professionals,
I am preparing a questionnaire for my summer internship project on the performance management system and appraisal of associates at a beer manufacturing plant. This is my first time working on a project like this independently. I have prepared a synopsis, which I am attaching with this post. Kindly review it and provide any suggestions that could be helpful to me. Additionally, please advise me on the types of questions I need to include in the questionnaire for two specific groups:
1. for HOD and team leaders
2. for associates
Your guidance on this matter would be greatly appreciated.
Thank you and regards,

Dear Mr. Simhan,
Thank you for your suggestion. I asked one of my seniors whether to write it in the past or present tense and framed it accordingly. But now, I will make the changes accordingly. Most of the parts in the synopsis are my own, apart from the introduction part.
I do not understand where is the question of sampling? There are a total of 93 associates in the organization, so I do not need to do any kind of sampling to choose the sample size. Please clarify my doubt.

Dear Nivedita,
I was referring to the Introduction, which is a cut/copy and paste job. "SAMPLE SIZE: Since my study was on the associates of HBL, and there were 93 associates on the payroll of HBL, therefore, the sample size for my study is 93." That's what you have written in your methodology.

Dear Nivedita,
I have seen your synopsis. Have you prepared your questionnaire? I am a faculty member and have been guiding PG-level students of MBA, MLS, and Industrial Relations for their projects here in Nagpur. If you need help, please feel free to write. Some of my students have recently concluded their project on performance appraisal.

Dear Colleague, Please, first try to make your own. There is a lot of joy and sense of achievement in making our own or trying for one. Then you may send it across others to add/delete/amend
