I am working in a company (the company are into building material) employed me as sales clark. I was given three month probational priod whice I was completed since January, 2012.
Now, I have discussed it with my direct boss vivally, and he said I should put it in write and apply for confirmation letter.
How can I write it. Please guide me.

Good day,
I am working with a company going to six months now and i was given three months probation which i have completed two months ago. I was waiting for my confirmation letter after completion of my probational period but I did not see any tracing regarding it, due to this I went to my direct boss complained to him and he asked me may be I have written to apply for the confirmation letter which I reply him no and I also complained to him about my salary that been lower than everyother staff in my same level, even junior staff is earning more than I do. My boss now said I should apply for confirmation letter and also include that lower salary in my request letter.
Please I want you to send me the format.
