If you have registered with the Labour Department, ESI & PF as per norms, there is a 100-page book available in the Law Stationery Book Shops. Alternatively, please follow the register you have to maintain in your office:
1. Attendance Register is called the Muster Roll.
2. Wages register will cover ESI & PF as well as the Labour Department.
3. Advance & Loan Register.
4. Deduction and Damage Register.
5. Inspection Book for ESI - this is sufficient for nowadays.
6. Inspection Book for PF.
7. Inspection Book for the Labour Department.
8. PF Eligibility Register like Form 2, Form 3A, and 6A.
9. Accident Register for ESI.
10. Bonus Register.
11. Overtime Register.
If you provide me with your business type, I will assist you in determining what should be maintained perfectly.

There is no specific format, it's just a 100-page register book where the labor inspector writes down the remarks of his visit as we have provided all the related documents that he asked for.

What Mala says is correct. There is no printed register available for ESI and PF Inspection. You have to keep a register and submit it to the inspectors to write remarks if any. That's all.
