Please advise us regarding the Forms to be filled and additional documents required for the withdrawal of the PF Amount in case of a death of a worker. The worker has expired while on leave.

well, in case of death employee's nominee or legal heir should apply in form - 20 (to claim provident fund),
form - 10 D (to claim pension) and form-5 IF (to claim assurance benefit under Employees' Deposit Linked Insurance '76)
and about esic, as you mention employee was on leave, his family can get funeral expenses but about other benefits, i have doubt they will get it.

In case of PF : Form No. 20- for Pf withdrawal by Nominee, Form 10 D- for benifit of pension of wife/children upto 2 No.
EDLI: for insurance benefit under EDLI scheme.
Documents required: Death certificate issued by District Authorities. If nomination is not done by the employee then forms should be supported by affidavit of nominee/legal heirs. Bank saving pass book of wife and eligible children(of same branch of SBI/Punjab National Bank or any bank authorised for pension purpose by PF authorities, 3 sets of passport size photographs of each eligible member under pension scheme, family details on company letter head, Date of birth certificate (in case of children issued by the school authorities), Certificate of company declaring that employee was in service of the company.
In case of ESI: At present, any family member can claim funeral expenses occurred on funeral as per limit. As employee death is not occurred in course employment family is not elligible for any other benefit under ESIC.

Refer to your query on the above subject, the dependents of employee's family will get the following benefits in the case of death of an employee in the course of employment.
1. Legal Heirs certificate (Original) issued by Panchayat/Revenue Authorities of the area concerned.3.Savings Bank Accounts of Nominees and Dependents 4. Family Members Photographs(Card Size) and Individual Photographs(03 no's each) duly certified by authorized signatory of the company 4. Family Members certificate issued by employer 5. Children's Bonafide Certificate issued by Educational Institution if they are studying etc., to receive the benefit from PF.
2. ESIC : In the case of ESIC , the dependents will get dependent benefit i.e.1/5th amount received by Nominee and remaining part of 2/5th amount received by Dependants depend upon the salary of the benefit period of the employee and also Funeral Expenses Benefit @ Rs.3,000/-from ESIC after submission of relevant documents as mentioned above.

A lump sum payment not exceeding Rs.10,000 towards expenditure on the funeral of a deceased insured person is paid to his nominee or either to the eldest surviving member of the family or if he has no family or not residing with his family at the time of death, to the person who actually performs the funeral of the decesed insured person.
The ESI notification is attached herewith for your reference.
Further, The nominee of the deceased employee will be eligible for gratuity if he has complected more than one year of service, and also eligible to get the funeral benefit from the labour welfare board

If an employee was unmarried and only Form 20 is to filled ( no need of form 10D)
