As the work is normally carried out by unskilled workforce, the effective supervision must be planned and executed to avoid such incidents/accidents in workplace.
We must learn from this accident and avoid such recurrence even in our workspot.

Hi,
I dont think the corrective measures or preventive measures given by you are propriate.
Need to think in other manner. The problem was not addressed properly, still the hazard is
not resolved if you have done the risk assessment of the activity please share becoz there are
lot of things to share.
See what you have mentioned is secondary things the investigation is incomple.

I have shared the incident report which I got from somewhere , hence not having any detailed risk assessment report...
Let's try to learn and implement the positive outcomes... So please come forward with your views... Requesting others also to participate...
