Dear Member,
Please mention under which enactment you want information about the mentioned form. If it for Form 5A under Provident fund and Mislaneous provisions Act 1952 then it is used to file Return of ownership to be sent to regional commissioner. (under para 36A of the Act).

I may add to what Shreekant has put. That is, form 5A under the EPF is a declaration to be filed by the establishment stating the names of owners of the company and the person who will be ultimately liable for payment of contributions, filing of returns etc. It will be on the person named in the form 5A that the EPF organisation will be initiating any penal proceedings in case of non payment of contribution and other offences.
