This is only a simple format. In this form item 6 to 14 & 16 to be filled by the person who supervises the particular employee. And items 1 to 5 and Leave/Absent to be filled by personnel/HR. If monthly Leave/Absent particular arefilled, Item 15 will automatically filled by itself.
