As you require the MIS Excel sheet, it would be better if you prepare the same at your end. Because it will help you to prepare other such formats too. I am describing the contents sheet-wise:
First Sheet-
Details of employee
a. E.Code b. Name of employee
c. Date of birth d. Designation e. CTC
f. Address for communication
g. Permanent address h. Contact number
i. Blood group j. Maximum qualification
k. Extra qualification l. Total Experience
Second Sheet-
Salary structure
In this sheet, write down the complete salary structure and make a column for the salary after any revision.
Third Sheet -
New Joinees
In this sheet, write down the names of new joinees of that month with details like CTC, Designation, DOB, etc.
Fourth Sheet -
Resigned employees
In this sheet, write the names of employees who resigned during that particular month with details like name, designation, date of joining, date of resignation, last working day.
Fifth Sheet -
Attrition rate

I appreciate the guidance you have given to Bhupesh. MIS in a recruiting company cannot be more than that. However, in the sixth sheet, one more column may be added "Exit Interview". This addition would assist the organization to exercise adequate control over attrition. :idea:

It's a good point that you have raised, but an Exit Interview is a different action that can't be taken in MIS separately; it can be included in the sheet for resigned employees. Just add another column and indicate whether the Exit Interview has been conducted or not.
I think it would be a better option to include the Exit Interview.

3. You have to calculate the rate of turnover in the company.

Just go through my points regarding MIS preparation once again. I am just clarifying your questions point by point.
You will find the details to write about new employees in the 3rd Sheet.
Regarding the rate of turnover, it is already detailed in the 5th Sheet under the heading Attrition rate.
For final settlements made during a month, you may add another column in the 4th Sheet with the heading "Status on Full & Final Settlement."
In my opinion, there is no need to mention other details like how much salary has been paid for Full & Final settlement. This is another statement that you prepare at the time of preparing Full & Final settlement, so there is no need to include that in the MIS.
