First of all, clarify, are you asking from an employer's point of view or an employee's point of view.

Some basic requirements are as follows:

The company must have more than 20 employees and be registered with ESIC thereafter.

The employee's gross salary should not exceed 15,000 per month.

After joining a firm, the employees need to fill a declaration form and provide a family photograph to submit to the employer.

The forms are then signed and stamped by the company and sent to the ESI office through the concerned person or consultant.

The temporary card is given within a week's time by the ESIC office to the concerned person or consultant.

The permanent card is sent to your address mentioned in the declaration form along with the ESI stamp on the photograph provided by you.

This permanent card is one-time and can be used with any employer through the transfer of employer.

The permanent card is invalid without Form 37, which is provided by the employer every six months, i.e., October and April. This card states that you are working with a particular employer.

I am working in a company registered in Delhi with ESIC. Some time ago, I asked our HR department for an ESI Card. I filled out the form provided by them and sent it along with my postcard-sized photograph.

They had the form attested by the ESIC department in Delhi and instructed me to visit the local office for the ESI Card. I am based in the Karnal Branch, and the local office for Karnal and Panipat (Haryana) is located in Panipat. Yesterday, I visited the local office and approached the concerned person to request the ESI Card. However, he requested an Insurance Number, which should be in 10 digits. When I asked our HR department to provide the number, they mentioned that the insurance number is 2013560301, as indicated on the form.

Unfortunately, the concerned person at the local office is not accepting this number.

Kindly assist me with this matter.

Hi,

After going through your letter, I came to know that your office has a registered office in Delhi. The company must have the ESI code at its Delhi office address. Since you are working in Haryana, your company must apply for a sub-code in Panipat, and then you can register yourself in Panipat.

Dear Mr. Rajekashyap,

Ms. Yaasmin has answered your queries correctly. It will be the employer's duty to provide the ESI card to its employees, and the employee need not go to the ESI office. However, you may support the concerned case worker and take the initiative to visit the ESI local office or branch office. If the ESI number is not available and in the case of industrial accidents, the employer will be held responsible.

I need my ESI card as I have not received it to date. I already have some knowledge about the ESI office, but they said the card has already been sent to the company's address. However, when we contacted the company, they confirmed that they have not received it. Please take necessary action.

Dear member, it is the insured person himself/herself to apply for the Duplicate Identity Card at the branch office to which he/she is attached. He/she may be advised to contact the appropriate Branch Office of ESIC and fill out a form for the issue of the Duplicate Identity Card (DIC). I believe there is a nominal fee (earlier it was Rs. 1 only), which the employee needs to pay to the Branch Office of ESIC. Overall, I think it is an easy process.

Dear All,

I have joined a factory wherein last month I have been enrolled in ESI. However, I haven't received my ESI card yet. A friend of mine has informed me that the procedure for issuing ESI cards has been stopped. Could you please confirm if this information is correct?

Sir, I think the system of issuing TIC or Permanent Identity Card for ESIC-covered employees cannot be stopped because such documents are required to be shown to authorities in ESI Dispensary/Branch Office/ESIC Hospitals, etc. Hence, I suggest you contact the concerned official in the factory dealing with ESIC or the Branch Office Manager of ESIC to which your unit is attached and demand such a document as mentioned above.
