Dear Srimurugan,
Factories Act, 1948 do not have any provisions relating to grant of marriage leave. Chapter VIII is there which relates to annual leave with wages.

In my view you want to know about forms & not forum under the Factories Act, 1948. In this regard it is clarified that Factories Act do not provide us forms. Forms are provided by the Factory Rules & therefore first examine which of the Rules i.e. Central or State is applicable in your case & then go through those Rules to have knowledge on these forms.
