I Want to leave my company without notice. Now i am transferring to another state. what can i do before leaving this city to apply for the PF without concern with the company (to avoid coming back to this city). Or can I apply from new city after transferring (by attestation from Gugitted and submitting to PF office.).

In transfer case, you need not to get it attested from previous company. Only new company has to attest it. You simply need to have your PF account no. of previous case.
In withdrawl case, it is mentioned on the PF form (form no.13) who are other authority to attest it otherthan your company (few of these are any gazatted officer/manager of your bank) and directly can be deposted to Pf office,but in all cases you need to have your PF no./date of joining and date of leaving.

For 19&10C one needs to fill it up & get it certify from the previous employer .It has to be submitted in the previous concerned office.
For 13 one needs to fill it up & submit to the present employer .The present employer will submit it to the concerned PF office.

Please contact the concern HR of your previous company about the delay in submitting your claim. Or please fill the form once again and handover it to your previous company HR dept personally and followup with them. Or in other case you can submit the PF claim form directly to the city PF office duly attested by gazetted officer / bank manager etc. This might help you.

if u want to withdraw then you collect a pf withdrawl form and submit to your old p.f. office with an attestation of your bank manager (old place). after submitting the same you will get the amount transferred to your bank account withing 2 months.
if u want to transfer the amount to your present employer, you should get signature of old employer. in your case it is very difficult. better you withdraw the money as per above procedure.
