First of all you need to decide you work on CTC Concept or Gross concept.
on that you have a policy stating that on CTC / Gross 30% shall be Basic.
Further, 40% on basic is HRA, conveyance fix it to Rs. 800/- as per the income tax rules. medical allowances can be fixed to Rs. 1250 pm. the residual allowance can be termed as special allowance or other allowance.
PF is 12% of the basic drawn as per statute.profession tax is as per the state govt gazette.

Dear chavakrishnasudha
Why do you want to prepare a 'single' payslip with "basic around 11000" and "for 30000/- per month"??
Unless you disclose the purpose and background of the matter; it would be difficult to get appropriate suggestions.
Regards.
