We have registered our firm under the Shop & Establishment Act on 21st January 2010. Could anyone please advise which form is to be filled and on which dates, along with the respective amounts? Our office is located in Gurgaon.
Additionally, please provide guidance on the mode of payment, the payee for the cheque, and the deposit location.

I have three companies located in different regions of India, and I would like to inquire about the following:
1. Does the LWF require any specific registration?
2. Can the LWF amount from Chennai be paid in Bangalore?
3. If the company does not meet the basic criteria regarding the number of employees in each location, is it acceptable to skip the payment?
4. Should the DD for eligible regions be paid at the LWF Office of the respective local regions?
5. Are there any supporting documents required for this payment, and if so, what are they?
6. Can the half-yearly payment be made annually? Will there be any interest incurred?

Not necessarily in every State. You can send local or at par cheque to the concerned Welfare Commissioner's Office alongwith relevant Challan-cum-Return by Regd. AD/ Speed Post. They will send receipt alongwith Code No.
2. Can the LWF amount of Chennai be paid in Bangalore.
No. This contribution is payable for the respective State.
3. If the company doesn't meet the basic criteria of the no of employees in each location is it ok to skip the payment.
Yes. But if you are carrying out any contractual job than the Principal Employer can insist to make the payment.
4. DD for the eligible regions has to be paid in the LWF Office of the local regions.
You can make the payment by cheque but with the LWF office.
5. Does this payment have to go with any supporting documents? what the documents?
Yes. With Challan-cum-Return. In Maharashtra Form Challan A-1, Bangalore Form D and in Chennai pls check with the LWF office.
6. The half yearly can it be paid once in a year? will this have any interest?
No. You have to pay half yearly.
However, in Karnataka the contribution is payable yearly i.e. the contribution is deductible in the month of December salary.
Employee's Contribution is Rs.20 per employee and
Employer's Contribution is Rs.40 per employee.
The above contribution shall be made along with a statement in FORM “D” prescribed under the Karnataka Labour Welfare Fund Act 1965 & Rules and submit it to the board on or before 15th January 2018 to:
