Good morning ! There is no difference of formats under ESI & PF Act State to State, bcoz these are the central Acts. Only the difference is of language. The Forms can be printed in the language known there.

Dear Prasad ur next question is for providing the PF fascilities to the person getting Rs.6000/- salary. In this connection it is to apprise you that if ur establishment is covered under the PF Act then you have to cover him under the PF Act.

Dear Kamchoor, if ur establishment is covered under the PF Act, then you must have maintaining the PF elegibility Register. Just put the name and other details of the new entrance employee in this register and allot the PF a/c number in this register it self serial wise. Fill up the Form-2 i.e. nomination form of that employee and attach the form with the Form-5 (i.e. return of new joiniees) and send it to the RPFC office within 15 days of the following month.
