I know that there are different ways, how to get a refer from an employee. But I hope there are some forms where these employees or candidates who come for the interview can fill up if in case they want to "Refer A Friend".
We are doing a "Refer A Friend" program in our company...wherein the employees refer their friends for an opening.
So, I just wanted to know is there any other ways or any other interesting template or form where I can implement and get more results.

I do not have a format as such, but can tell u as to what information to put in it.
You can add the requirements for the designations / posts alongwith some incentives depending upon the designations.
e.g. Post of Assistant Manager,
Qualification- MBA or so and so
Incentive - if selected 20,000/-
