In ESI form 37 what we would write in that blanks.can any say which dates would be written in that blanks........
(i) has continued to be in employment /re entered insurable employment on......................... and contribution have been payable/paid in respect of him/her during the contribution period which began on...........................
(ii) has paid contributions for 78 days in the preceding contribution period which ended on.................................

first blank you should mention date of joining , second blank you should mention current contribution period starting date(1st oct) and third blank should be filled as previous contribution period end date(30th sep)if he has contributed more than 78 days.if not then it is not applicable.pls put cross mark for that point.i hope I cleared your doubt.
