Dear Maneesh,
Employee hand book contains policy and procedures of company so it should be very specific and is always different company to company according to practices in company.
Anyways, I am herewith attaching a sample, I prepared my company's employee hand book taking help from this. (Sorry, I am not attaching my one because of confidentiality). But you will certainly get an idea from the attached one.

If this reply was directed at me, go back and read what I said. You asked for help, I gave it to you. There are dozens and dozens of Handbooks posted here on CiteHR. The topic has been brought up many many times.
Sadly, one must assume that you cannot be bothered to do some basic research and just expect other Cite members to do your work for you and provide a ready made solution.
You will learn nothing that way and it will not help you grow as a person or a competent employee for your organisation.
