I have chased with my previous employer for PF withdrawal amount, but they are not responding back even I have sent PF withdrawal forms to them number of times but all my efforts gone waste, now I have checked online (PF Site) that if your employer doesn't respond to you or they are not doing anything then you can send us the PF withdrawal form with attestation of any gazette officer.
Now the issue is where I should send this form, because I am not aware which one is the right PF office, I have tried my level best to get to know from my previous employer that from which office they are having an account. (This office is in Gurgaon and my PF account starts with ‘DL')
Anybody can help me out there.

They might have taken the code from Delhi, and you should know your account number. Do one thing: fill up your form with the company name and at least get your account number and deposit it in the Delhi RPFC office with an application stating that your previous employer is not attesting the same. It is the duty of the employer to attest and return the same to the PF office within five days.
