- Total # of years of experience
- # of years of relevant experience
- Technologies / skills worked in (use a rating scale of 1-5 or evaluate on levels – beginner, intermediate, proficient, expert)
- Education (from SSC till the highest level) – check for year of passing, percentage, grade, course / specialization, university, institute, etc.
- Work experience (all the companies worked so far) – duration, salary drawn, role & responsibilities, reason for leaving, probe on the company and URL if not known
- You can also get a list of basic questions from the technical panel with answers for initial screening
- Check for salary details (CTC, Gross, Take Home, Expected), Notice Period (official and how much it can be reduced, is a buy-out on notice possible?
- Check for any bond, duration, contents of the bond agreement if any
- Relocation – how comfortable is the person in relocating if based outside.
- A brief on the family

Based on the answer he provides for "reson behind looking for change" and his " interest to wards the position u offered him", that interest you can identify from the way he respods to your call and questions he ask regarding the position. These gives u a picture on the 3 points u had mentioned.

You can request the candidate(s) to take you thru the last 2 projects that he / she has worked on. This would give you a fair idea of how he has performed as a Team and you will also get to know his accomplishments.
You can also prepare a skill metrics before calling the candidate(s) and check it with them which will give you a fair idea if the candidate has really worked on the skills that has been mentioned in his / her Resume.
