Annual recruitment cost = the total consultant fees + cost of portals + cost of recruiters + cost of advertisment + cost of referals.

Annual turnover of the company has no linkage with Recruitment cost.
The formula is quiet simple for Average recruitment cost.
Average cost of Recruitment = Total money spend on recruitment for the period (month or year) / Total number of employees hired in that period.
The calculator shared by AJ is quiet comprehensive and you can use that as well.
