Dear Neha,
You could have given little more information like nature of your industry, your designation etc. What kind of outline you require? Do you want outline of the training programme on Mentoring or outline of the "Formal Mentoring Programme" itself?
In the course of my training career, I have handled training on "Formal Mentoring" several times. Considering my experience of my past clients, I can say that first you need to devise the "Policy on Formal Mentoring". Your Mentorship programme should be derived from this policy.
Formal Mentoring is not just making pairs of mentors and mentees and leaving it them. You need to train mentors and mentees and further you should have someone who monitors the programme is executed as per plan or not. If mentoring is not properly, it could result in conflict thereby making more harm than good.
I have given exhaustive reply to one of the past posts on this subject. You may click the following link to refer it:

In this link, you will find several other links. These are also my replies to the queries of the past participants. You may go through all these links patiently in order to understand the subject better.
If you wish to train your staffs on training, then you may approach me. I will help you in devising policy on Mentoring.
Thanks,
Dinesh Divekar
