Hi, One of my employee died from COVID 19, As per ESIC one can claim for Relief under ESIC COVID 19, Relief Scheme. The form have been filled , my query is to know what all supporting documents need to be attached along with form like covering Letter, master roll certificate, Death Certificte, Adhaar Card Copy, Bank account Passbook Copy, Birth certificate of minor dependant.
Do i need to attach Photographs of surviving Family Members & List of surviving family members in death cases, If yes that what should be the format of the same.
Please guide Seniors.
