Hi,
About this form, include the following:
In Date / Out Date
In time / Out time
In Location / Out Location
Apart from that, I dont think you need this format at all when you are using the bio-metric machine.
Even if employees forget the Card at home, still they can punch the attendance via machine.
Which machine you are using beacuse now a days most of the bio-metric machine provide multiple systme to mark the attendance. Like
a) Punch Card/Swipe Card
b) Thumb
c) Pin (Just Like ATM Pin)
So if employee has forgot/lot the card, he still can enter the PIN and mark his attendance.
I have this kind of machine with me. Now, you can get all the exception reports like who all used card, who all used PIN and who all used thumb.

Do you really need formats for this? We just get an email, which the employee addresses to HR and marks a copy to his HOD, giving his IN/OUT time. Generally he mentions "forgot swipe card". If this is frequent, one can investigate if there is loss of card etc.
