You can do the employee satisfaction survey based on the following:
1. Prepare a questionnaire for each level you want to measure the satisfaction.
2. Do an interview either as a whole group or as a individual.
3. If required you can hire external person in doing this survey as the employees might come out with more grievances if they have any.
4. Try to meansure this based on the scorecard of each survey independently by taking the feedback in case of low scores.

Generally you have to co- relate certain things and calculate the Employee satisfaction level.
We generally used following methods for calculation employee satisfaction-
1. By questioner
2. By Fluction rate per month.
3. Days of Sickness.
4. Accidents taken places in each month.
5. No of Ideas posed per month or year by per employee.
6. Monthly no of training hours attended.
for which you have to prepare some graphs and calculate employees satisfaction.

For calculating the satisfaction level in the organization you need to define the sub parameters for measurement like
Satisfaction with work environment
Satisfaction towards management
Satisfaction towards facilities company is providing
Satisfaction towards higher authority
Satisfaction with the collogue etc.
Also keep place to notify the dis-satisfaction area.
Accordingly you have to prepare the survey questionnaire and conduct a pilot survey.
See the results and then form the final questionnaire.
