Form 10 is to certify by the employer that the insured person has not worked during certain period due to medical reasons.
It cannot be mentioned till date, please verify it with the ESIC official.
It should be mentioned as exact match with the leave dates of the insured person in employer leave records.

Madam: but the ESI officer state that there is nothing to bother about approving till date which was mentined by authority.the leave may extent further so that mentioned as till day.what should do now.

Has the employee reported to duty? I hope you have submitted this form for processing his sickness leave wages.
If this is the case it cannot be mentioned as till date as the employee already reported to duty.
