Thanks for clarifying that it's for a student's project. Is it a joint project with another student?
Why are you asking for personal info (e.g., name). It's always better to clearly indicate why you are asking and what use it will be pu to. Also, it is better to ask these questions at the end rather than at the outset.
What makes you think that employees of the 3 companies will be accessing this website, which is specialist site for HR personnel.
Do you not think that you are expecting people to spend their time downloading, filling, saving, and then send the same as an attachment to you by email? It would be more appropriate if you use a web-based survey. If you search CiteHr you will find some examples.
What is the purpose of collecting from these companies? Who will be the benficiary? If it is for your final MBA project, then it may not allow you to exhibit your knowledge of statistical tests.

The informaton asked in your survey is way too confidential. Do you really think someone would or should reveal their base salary / compensation details / bonuses.. this information can be used for illegitimate purposes. Is that what they teach you in MBA colleges !?
I would strictly recommend everyone in this forum not to go for such surveys and protect yourself and your organization from any potential threat of misuse of your info.
