Confirmation is a process that initiates from your reporting head's desk to HR (OR HR reminds the TL that the employee has completed 6 months of probation; Is his service to be confirmed or to be extended). You need not write a letter for the same. In case you feel that the same is not initiated, you can remind your reporting authority about it verbally. If he insists you to write an email then n then you should do it that way.

Pls check with your HR-department that is any format for to be confirmed employee, fill it and submit to your immediate manager if is there.
or else just talk to your manager on your probation period , confirmation and salary increment.
i hope salary increment will happen once in year and same to check the performance policy.
