Excellent presentation and, I really do not why these types of figures are not be published. Dear friends, this is an eye opener to everyone and in my opinion after serving 37 years in industries in various capacities, if people are not ready to see and correct themselves, I am afraid, how these people will handle tough situations like this.
Hence, gentlemen, please accept the reality of life and try to learn from others mistake so that you can save yourself and your colleagues.
In my opinion, many like this to be made available for public so that they can train their people from this and lead and safe life.
I wish Shailesh, all the best and keep posting presentations like this for the benefit of others without worrying about the other people comments.

I agree with what Mr. Narayanan & Abhay have explained. Such presentations are an eye opener for each of us.
Why we want to avoid seeing such practical incidences ? This can happen anywhere where safety rules are bypassed. We must learn from this & shud take necessary steps / actions in our respective organisations to avoid this happening.
Thats why in some good organisations a safety officer inspect & verify the every new incoming M/c or equipment for required safety features to avoid such incidents.

In industry and service sector one should not indulge into the repairing / maintenance / prevention of accident - like activity unless and untill he is fully experienced. At times out of ignorance on this part and out of over confidence one has to be put into the strange situation causing personal loss, mental agony to family members, organisation where he is working.

This is really horrible incident which no one wants to share, but you have done a good job to spread safety awareness amnog all peoples who are not directly related to such kind of jobs.
