You could have searched the policy on your own, amended it and then could have uploaded it for review by the seniors. You wish to be provided with a ready-made policy. Are the members not supposed to do the spadework?
Seeking a ready-made policy apart, the quality of your post is far from satisfactory. Members are expected to do spell check and grammar checks before they upload the post. The casualness of the members casts in poor light not just the individual member but the HR fraternity as a whole.
