Memo /warning letter should contain all details and you can't blame a employee just like that.

This is in addition to what a learned member has said previously.
If there is a breach of the code of conduct and if you wish to impress upon the staff to desist from such a breach in future, then you may issue an office circular. "Memorandum" which is called "memo" is short and is generally issued to an individual. It has a negative connotation and it is issued to seek an explanation. Occasionally a "warning letter" or "show-cause notice" is also called a "memo". However, a memo cannot be issued en masse.
If you wish to office circular, maintain a formal tone, and write what happened and where happened, however, you may withhold the name(s) of the person(s) involved in the breach of the code of conduct. Write whether it was a one-time incident or there were repeated incidences. Thereafter, write whether a violation of any policy in vogue has taken place.
In the next paragraph, write what were the consequences.
In the last paragraph write that the staff concerned should desist from violating the code of conduct. The breach will be viewed seriously and a strict disciplinary action will be initiated against the defaulters.

You can issue a warning letter to the employee indicating his wrong doings and the things should not be repeated in future or adding the lines, if there is no improvement from your side than the company can initiate action as it deem fit.
