What is the thumb rule for From 11? If the person Gross salary is to be Above 15000/- or basic salary is to be above 15000/-.

Form 11 is for an employee who joins you terminating the employment in some other firm where he was a member of EPF a/c and contributing. He/she is a new joinee taking up employment in a PF covered establishment. As per para 26(6) of the PF Scheme, once an employee is covered under PF, he/she continue to be a member irrespective of the salary exceeding the wage limit under the Act. So when this employee is granted increment by which his 'salary/wage' exceeds the thresh hold limit does not matter, but whether to contribute on full salary or limited to the wage ceiling is an option available to both employee and employer. Normally PF contribution is limited to Rs.15,000/- but employer & employee both can exercise their option for over & above this limit as well.

As you said that, once an employee is covered under PF, he/she continue to be a member in new establisment covered under EPF. Please read
as per EPF Scheme, Para 69, (5) - Any member who withdraws the amount due to him under subparagraph (2) shall, on obtaining re-employment in a [factory or other establishment] to which the Scheme applies, be required to qualify again for the membership of the Fund and on qualifying for membership shall be treated as a fresh member thereof.
It means on his re-employment, he should required to qualify for EPF. so if his salary is more than 15000/ then he may not be EPF member in new establishment.
employer may allow to make him as PF member but in joint request in writing .
