i need matter for staff to ask why he was absent 1 week from work without any information my manager asked to give letter to staff regarding why he was absent and that we need explanation. I need format and matter please help.

It has been reported that you remained absent from your duties on from xxx to xxxx date without sanctioned leave or permission of the management.
You are aware that your sudden absence from duty disrupts smooth operations and causes operational difficulties, also your above act of remaining absent from duty or leaving the work spot is violation of company rules and the management takes a serious view of the same.
You are directed to explain in writing as to why disciplinary action should not be taken against you.
