DOJ cannot be missing, but the employees who worked before June 2014 and left service, their records may show it as missing. In such cases, the allotted UAN number also remains with the employer only, and the employee cannot obtain it without communication from the employer. In this scenario, Form 5 and 10 are needed from the concerned employer. An alternate option is the submission of a joint declaration from the employee and employer, which would suffice. The last option is to file a manual Form 13 with own calculated DOJ & DOE duly signed by the present employer for transfer. Subsequently, the department will request the previous employer to confirm it. On the other hand, they can also contact the member to provide the source of such available dates. The preferable option is to rectify the situation with the assistance of the previous employer.
