Please go through the State Factories Rules and then find out the formats of different returns. Mainly, the following returns are submitted to the Chief Inspector of Factories:
- Form-21 Annual return - General
- Form-31 Annual Return - Accidents
- Form-22 Half-yearly return
These returns relate to Haryana. So, I advise you to refer to your State Rules.

In Haryana, we submit the half-yearly return in Form 22 (ending on 30th June only) under The Punjab Factory Rules, 1952. Therefore, please review your applicable Factory Rules to determine the Form number.
