I am Ellie, a secretary in an Educational Institution. Our HR Officer, directs me to compose or draft a survey questionnaire for him because right now, he's having his dissertation about the Performance Management System of our Educational Institution: As basis for Proposed Intervention Program.
In this regard, I want to ask for your permission, if i could use your questionnaire as reference, so that, I would be able to draft his questionnaire.
This is really hard, I admit, but thanks to this website, i am learning a lot. I am just disappointed with our superior because he is designating others to do the job for him, even if he should be the one to research about the topic. This is a dissertation, he should know this study more than anyone else...

I am amazed that your HR officer is expecting you to do his private work (exercising his power or misusing his position).
I hope he gives due credit to your effort in the acknowledgement section of the dissertation report.
