you can prepare that form on your own. it must include:
Name
Current address
permanent address
contact number (mobile and landline)
space for photograph (postcard sized)
highest education
board of education
Last two or three experiences with given details:
. Organisation name
. job title
. gross salary/CTC
. reporting structure
. name of last supervisor/reporting authority
. whether that authority can be contacted or not
. if yes
. contact number
. reason for leaving that organisation
. languages known
. other technical skills
. reference
. date
. signature

Please suggest what plans should I adopt in the beginning to operate as an Admin and HR Manager besides recruitment of staff etc. How should I make it a successful department at a shortest possible time?
