Questionary regarding the welfare activities in the company:
1) How many workers are working in your company? If more than five hundred during the preceding year. Have you employed a Welfare Officer?
2) Are more than 1000 workers are employed in your company. If yes, have you appointed a safety officer?
3) Have first aid boxes been provided in case you have more than 150 persons?
4)Are you giving creche facility to the lady employees? (As per Factorys Act, 1948, Creche facility is provided where 30 or more ladies are working).
5)Canteen facility is being provided by the company in case having 250 employees or not.
