Absenting yourself from duties without prior intimation is a misconduct for which you are making yourself liable for necessary action.
You are hereby warned to refrain from such activities; failure to do so shall invoke appropriate action.
You are further advised to submit a written explanation on your unauthorized leave as soon as you receive this letter or as soon as you resume duties.

You can try this also.
Dear ------
It has been brought to our notice that you are absenting from duty since ………, you are therefore called upon to report on duty latest by ……..along with an explanation for your unauthorized and continuous absence, failing which it shall be presumed that you are no longer interested in the employment and have abandoned the same.
Accordingly, the management shall initiate necessary and appropriate action including removing your name from the rolls of employment.
