You have to submit a duly filled joint declaration form,. You have to mentioned your wrong DOJ & correct DOJ on that joint declaration.
You can process it through the current employer or previous employer.
When you will submit FORM-13 (REVISED) your form may be reject from PF dept. but you can try 2- 3 times, your application will be approved so nothing to worry.
