I am about to complete my probation period and are told to submit my self performance evaluation sheet. How should i do that? please suggest

What is your designation? You work in what department? What was the length of the probation period?

Generally, post-probation evaluation is done by the senior authorities of the company, which, of course, includes the HOD as well. This evaluation is for evaluating whether the recruitment of the candidate was suitable or not. The evaluation uncovers the shortcomings in the recruitment practices if any.

However, you have been told to do the self-evaluation. This is a strange practice. It appears that the HR department has not provided you with the standard form for evaluation. If they had provided you with the form, then this query would not have come up.

Anyway, you may write the following:

a) learnings made during the probation
b) suggestions, if any given for process improvement during the probation
c) did you increase or decrease any ratio during the probation?

The following points are for "self-evaluation" as such, however, you may include them:

d) is there any scope to improve internal/external customer satisfaction?
e) have you observed that any ratio or cost is not measured in your department?
f) what support do you expect in near future from your colleagues in general, and HOD in particular?
