I want a covering Letter format for our customer to issue a new cheque against of old cheque. Anybody can help on this?

You have not mention why you need new cheque? what happened to the old cheque? Whether old cheque validity is over? Or it is teared? Or any other reason?
While drafting any letter it is always better to give brief summary or case history. So that matter is clear and you can easily submit the requirements.

However, pls find a short draft letter. You have to furnish information at required (===) places.
Moreover, you can make necessary changes as per your specific requirements.
