Dear All,
I am looking for a format for evaluating trainees/ participants after they attend a training program. Normally, we evaluate the trainer, the quality and relevance of the training program. If any one has a form to evaluate a trainee either immediately after the training program or his behaviour on the job, please share it.

Thank you so much for posting this query. Good to note that someone is attaching accountability to learners as well. Most of the time the accountability is towards only the trainers, that trainers should have this body language and he/she do this or should not do this. Whereas learners are as cool as cucumber.

The best thing for you is to conduct post-training test. This test could be immediately after the training or may be after week or fortnight. Any training conducted without test is bound to fail that large number of HR do not understand. By passing this step II of Kirk Patrick model of training effectiveness, they jump to step III and try measuring the training effectiveness. Most of the time it is just eyewash.

In my many trainings ( but not all), I send post-training faculty report. In the report, I not necessarily give feedback on each and every participants but include noteworthy points. The feedback is on who had shown creativity, enthusiasm in learning, who was laid back etc.
