For that you please approach the commercial tax department of your area. Earlier there use to be the manual form but now I am not sure. Other places everything became online & about TN I am not much sure.
The documents you require will be as follows
Address proof of the Company
Company card copy,
AoA/MoA or Partnership Deed or propreitorship registration document,
All Directors/Partners ID as well as Address Proof.
