I am working as a Sr.Hr Exe. in small IT company,employee strength is between 40-50. I have to prepare a Performance Evaluation template(format) & a questionnaire for yearly performance appraisal.
It should be categorized in three evaluation tests.
1) Employee Self Evaluation
2) Managers Evaluation
3) Hr Evaluation.
I have to prepare questions for all the three categories.
I would be needing help & suggestions from all the hr professionals of Cite Hr. to prepare the same.

I would suggest to go for a objective form rather than subjective one otherwise measurement part will be missing and in a long run that will affect the organization.
Design 1 simple form with the parameters and then 3 columns (self score, manager score & HR score).
Make it as fair and transparent as possible. follow the below mentioned steps:-
1. employee will fill the form by own
2. with the filled form employee will meet the immediate manager and they will discuss on scores and manager will fill his applicable column of score.
3. if there is a difference in opinion between both of them HR will enter and in a joint discussion of all 3 of them, HR will fill the final scores.
4. take the average / of it.
5. follow the bell curve for any promotion / grade change and increment
