Dear Sirs,
I am John Paul working as HR Executive in one of the retail industries in Bangalore. We have received EPF form 3a which is in different format when compared with the old formats used earlier. Please find the format attached herewith for your kind review. The form is in horizontal format. Contribution period starts like Wage4,APRER, APREE and PF4 (From Column 6 onwards) I understand that APRER & APREE are employer & employee contribution during April and PF4 is total of both. Similarly the calculation of contribution goes till March (Wage3). I don't understand what is meant by VOL_PEN as given in column 5. Can you please help me fill the last 8 columns also? Can you also guide me how to complete the entire form?

The format which you have is not the right one , you could get the 3A attached here,
and moreover from this annual year you doesn't need to prepare and submit 3A forms, because now it is online, you can automatically generate 3A from E-Tool software.
